Use these descriptions to find the best way to report the incident

Use this form to report potential violations of the Code of Student Conduct involving students or recognized student organizations. Incidents include but are not limited to:

alcohol misuse, drug use, violations of Residence Life or other campus policies, physical assault, organizational misconduct, disruptive behavior, or theft.

Use this form to report incidents of academic misconduct.
Academic misconduct includes, but is not limited to:

  • Cheating
  • Plagiarism
  • Use of unauthorized sources
  • Prohibited behaviors noted on a class syllabus.

Anyone in the Howard College/SWCD community who feel they have been subjected to unfair treatment by an individual or department are encouraged to submit a complaint. The concerns could include misconduct, or a condition believed to be unfair, inequitable, and/or discriminatory. Howard College/SWCD is committed to appropriate resolution of complaints as efficiently and effectively as possible. We believe providing a mechanism for complaints and/or feedback to be heard gives us the opportunity to improve our services.

Any Community member (not a student of Howard College/SWCD) who feels they have been subjected to unfair treatment by an individual or department is encouraged to submit a complaint. The concerns could include misconduct, or a condition believed to be unfair, inequitable, and/or discriminatory. Howard College/SWCD is committed to the appropriate resolution of complaints as efficiently and effectively as possible. We believe providing a mechanism for complaints and/or feedback to be heard gives us the opportunity to improve our services and better serve our college community.

Use this form to report any type of behavior that could be deemed a threat or concern to the Howard College/SWCD community.

  • A threat can be verbal or nonverbal. It can be communicated orally, in writing, through gestures, or by any other means, including electronic transmission. It can be communicated directly to an intended recipient or through a third party.
  • Behaviors do not have to be violations of law or college policy to be of concern. Community members are encouraged to report incidents of concerning behaviors that may not qualify independently as a threat. Instances of changed behavior that may singly or in combination generate concern include, but are not limited to:
    • Suicidal warning signs
    • Loss of temper/excessive irritability
    • Drug and Alcohol Use
    • Change in mental, physical, and social health
    • Waving a gun or knife (Call 911 then Campus Security)

Use this form to report incidents of sexual misconduct and behavior that violate Title IX.

This form is intended to be used as an option for a student or an employee of Howard College to electronically and anonymously report an allegation of sexual misconduct committed against or witnessed by the student or employee. This method of reporting is not for emergencies or crimes in progress. Always dial 911 for emergencies.

When submitting this form, it is important to understand you are not filing a police report. Sexual misconduct / assault can entail physical, verbal, written, audible, visual and/or electronic activity and includes but is not limited to sexual violence; sexual harassment; sexual exploitation; dating/intimate partner violence; domestic violence; stalking; sexual coercion; and sexual exploitation as defined by state and federal law and Howard College policy. Please provide as many details as possible to assist with the investigation process. When submitted, this form will be sent to the Title IX Coordinator.
