  • Registration Form
  • Student Agreement
  • Capillary and Venipuncture Release Form
  • Copy of Government Issued ID
  • Copy of Social Security Card (Must match name on ID)
  • Copy of Health Insurance Card
  • CPR Certificate
  • Drug Screen through castle branch
  • Background check through castle branch
  • Diploma
  • Shot record
    • Immunization Requirements
      • FLU
      • COVID
      • HepB (3 doses)
      • Vericella (2 doses)
      • MMR (2 doses)
      • TDaP (within the last 10 years)
      • TB Test

Application steps

  1. Print out application packet (link)
  2. Fill out and turn in paperwork
  3. Turn in required documents
  4. Once approved, you will be enrolled and sent an enrollment confirmation
  5. Pay Tuition

Tuition: $

  • Tuition includes building fees, exam fees, instruction, and clinical fees.
  • Both sections of this program must be complete to take the state exam

Lecture

  • HYBRID – online and in person classes within this program
  • Evening in-person class once a week – everything else is online

Upon successful completion of the program
