Program requirements

  • Registration Form
  • Student Agreement
  • Government Issued ID
  • Social Security Card (Must match name on ID)

  • Letter of Employer Responsibility
  • Background check through castle branch
  • Drug screen through castle branch
  • Form 5523 – Must be notarized
  • Form 5534 – Must be notarized
  • Must be a CNA and have worked in a facility for at least 6 months

 

Training Requirements

  • Classroom: 144 hours
  • Prerequisites: CNA (6 months experience)

Castle Branch Instructions

  1. Go to https://mycb.castlebranch.com/ to start the drug screen registration process.
  2. Press “Place Order” (By your name in the top right corner).
  3. Enter HH86 where it says package code.
  4. Agree > Fill out form > Submit > Pay.
  5. Print out drug screen voucher and take it to: W.T.I.P 1113 Scurry St. Big Spring, TX.

Application steps

  1. Print out application packet (link)
  2. Fill out and turn in paperwork
  3. Turn in required documents
  4. Once approved, you will be enrolled and sent an enrollment confirmation
  5. Pay Tuition

Tuition: $805

  • Tuition includes building fees, exam fees, instruction, and clinical fees.
  • Both sections of this program must be complete to take the state exam

Eligible to take the state certification exam

You must have all required documents and your MA application submitted to TULIP before you can sit for the MA Exam.

 

The State of Texas has to approve you. Once you are approved, you will be informed of the date, time, and location of the exam.

 

You must bring your ID and a pencil.
